This questionnaire provides the most cost effective method of evaluating your immigration status or concerns. It provides us with information necessary to advise you on how to proceed in your specific case.

PLEASE UNDERSTAND that the information supplied through this questionnaire will be used for the sole purpose of analyzing your immigration situation and preparing any immigration and entry documents that you may require.

INSTRUCTIONS: After you have read our Consultation and Payment Policy and the information above, please print the following questionnaire from your computer and fill it out as completely and accurately as possible.  Upon completion of the questionnaire, assemble any additional documents that you may use to supplement the questionnaire.
